3. Important Notes 

- Submitting the form with a requested locker number does NOT guarantee that you will be assigned that locker, though we will try our best

- After the form is received, you will be contacted by the GSIS office OA team confirming which locker has been assigned to you 
. - Please refer to the application form for detailed information (locker location, etc.).

- Return Date: Dec. 21, 2026 

The Office will dispose of remaining items after the return date. Please note that items in lockers not cleared out by the return deadline will be disposed of at our discretion.
